At Nurturance we bill for all motor vehicle claims (MVC).  If you have been injured in an auto accident you are eligible to receive massage therapy for the injuries related to the accident for up to a year after the date of incident.  All that is required of you after discussing your treatment options with your primary care physician or chiropractor is to bring in a prescription for massage, the insurance information (name of responsible insurance company and their address, adjustor's name and FAX number) and the claim number.  We take care of the rest.

I am a contracted provider for ODS Health Plan, Lifewise Health Plan of Oregon and Healthways Whole Health Networks (HWHN), and Uniform Medical Plan (UMP).  I no longer accept ASH or Health Net insurance.  If you have health insurance that is not listed on this page and you know that you are covered for massage therapy, please call or email me with your name, subscriber number, date of birth and contact number for your insurance carrier and I will check your eligibility requirements.
