Intake Forms

1. Download appropriate word document or PDF plus the business policy form.

2. Fill it out

3. Save it to your computer under a different name (for example, jane smith intake form)

4. Send it back as an attachment to barbara@nurturance.net at least 24 hours before your first appointment. This allows more time for bodywork with you if I get the form at least a day before your appointment.

5. Or you may and bring it with you to your first appointment

Business Policy   (word document) All clients: Please sign and date the business policy and bring it to your first appointment.

General Intake Form  (PDF form)This form is for first time clients who are not receiving the Maya Abdominal Massage or Chi Nei Tsang. Print out this form and bring it to your fist appointment.

Initial (MAM) Maya Abdominal Massage Intake Form   (Fillable form) The intake form must be filled out and sent back to Nurturance prior to the day of your appointment.  Important: Some people are unable to save this form after they fill it out, so run a test before filling out the whole form.  Save it under your name and email it to barbara@nurturance.net at least 24 hours before your appointment or print it out and bring it to your first appointment. Male (MAM) Intake Form  (Word Document) Men, please fill out the male intake form in addition to the main Maya Abdominal Massage intake form if you have a Maya Abdominal Massage or Chi Nei Tsang appointment.
