2. Men- Initial Intake Form  Please bring the completed form to your first appointment.

3. Motor Vehicle and Personal Injury Forms  If you have been injured in an auto accident you are eligible to receive massage therapy for the injuries related to the accident for up to a year after the date of incident.  All that is required of you after discussing your treatment options with your primary care physician or chiropractor is to bring in a prescription for massage, the insurance information- name of responsible insurance company and their address, adjustor's name, FAX number and the claim number.

4.General Intake Form This  form is for first time clients who are not receiving the Maya Abdominal Massage or Chi Nei Tsang. Print out this form and bring it to your first appointment.

5. Restorative Exercise Form Please bring this completed form to your first Restorative Exercise appointment.
